WALK-IN MEDICAL CLINIC SERVICES:

Our Nokomis site has a free walk-in medical clinic which is open to those patients that meet our previously cited criteria.  The clinic is open one day a week, Wednesday 5:30pm–7:00pm 

by appointment only. During these clinic hours, our dispensing pharmacy is also open so patients usually can obtain their medications and begin their therapy immediately.

Our organization utilizes volunteer physicians, nurses, pharmacists and patient advocates to accomplish these free services.

PHARMACY SERVICES:

The pharmacy dispenses free selected generic medications to preapproved clients from our on-site pharmacy on Tuesdays. The pharmacy also dispenses to our walk-in medical clinic patients on Wednesday evenings. Our on-site pharmacy also provides free over-the-counter products to qualified patients. Items include cough and cold medication, pediatric medicines, bandages, adult diapers, etc.

GSPHS also provides free health screenings, blood pressure measurements and glucose testing at a variety of community outreach sites.

APPLY FOR SERVICES:

Patients can apply for services by calling 

941-445-5687 for pre-screening to determine eligibility. Alternatively, patients can download a GSPHS Patient Assistance Application from this website. Use the link below to download an application form. The application can be dropped off at the Nokomis office or faxed to GSPHS at  941-445-5688. Once your application has been submitted a volunteer will contact you.

A few helpful points:

  • Clients need to be a legal resident and have a photo ID.
  • Clients must have no medication or prescription insurance.
  • Client must have proof of income. Proof of income may include: Last 4 weeks pay stubs, Social Security Benefits letter, Zero Income Letter and Letter of Support, etc. (volunteer advocates can assist in getting these forms if needed). 
  • Copy of latest tax return or non-filing letter.
